We provide clients a 'Supported Living' pathway from hospital or care home settings, back into the community. Our initial high support enables them to gain skills and confidence to move on into their own homes, where they will be able to thrive and manage daily life with the appropriate ongoing lower-level support or section 117 after-care.

We pride ourselves in our excellent success rate in rehabilitation of many service users with extremely complex needs and a past history of multiple mental health hospital admissions. Our flexible care packages offer shorter term, medium- or long-term placements as appropriate for client's specific assessed needs.

 

Our experience has shown that our client group usually requires limited regulated “Personal Care” and we have therefore adapted our service accordingly, in partnership with Commissioners and market needs. CQC Regulated “Personal Care” needs are provided by our outreach domiciliary care team who have maintained an overall 'Good' rating in latest inspections.

Mental Health

Our focus is on recovery and re-enablement to empower clients on their pathway of integration back into the community. By encouraging them to develop their skills and abilities, we help them establish coping mechanisms and resilience.


The clients we support may suffer severe and complex mental health difficulties, which may be impacted by a multitude of diagnosis. This may include schizophrenia, anxiety disorder, depression, personality and/or mood affective disorder, schizoaffective disorder, self-harm or suicidal ideation combined with learning disabilities and/or substance misuse.

Our Approach

Each Service User’s circumstances are unique and so is their personal
journey to independence and recovery. However complex the needs of the client, we will support them in finding happiness and gaining successful outcomes.

By focusing on providing a warm and welcoming atmosphere, we create a therapeutic environment. Our homely setting allows clients
to feel more comfortable and relaxed, therefore leading to better, engagement, encouragement and support.

 

With appropriate empathy and understanding, we aid each service user in building confidence and starting their recovery, after
their traumatic experience of a mental health crises.

Recovery Pathway

Recovery is a journey. Through our outcome focused approach, we support clients to engage, enjoy and embrace the simple things in life. We help them to develop new ambitions and achieve new personal goals.

 

We use the Recovery Star as a tool to create a 
recovery model for each service user. This puts focus on the positive outcomes and achievements for each individual client, along their recovery pathway.

Meeting Complex Needs

We are experienced and able to support people who:

​

  • Are moving on from a long stay Hospital or Care Home admission

  • Are subject to a Community Treatment Order (CTO)

  • May be subject to section 25a, 37/41 of the MHA and/or section 117 after care or MAPPA.

  • May have a Personality Disorder, Autism Spectrum Disorder and/ or a Learning Disability, Vulnerability or Mental Capacity limitations.

CQC Regulated Support

Provided by our outreach domiciliary care provider “Sapphire Care and Support Services” which includes:

  • Personal Care where required

  • Medication Administration and Management

Our 24/7 Non -Regulated Support includes:​​

  • Practical life skills development and re-enablement

  • Managing personal hygiene and self-neglect prevention.

  • Progression towards blister pack self -medication and medication management if appropriate.

  • Managing physical health needs including GP, Dentist and Optician

  • Statutory Clozapine Management

  • CMHT depot injection, bloods and review appointments

  • Personal budget skills development and personal finance management

  • Tenancy management and housekeeping skills

  • Educational and working skills development towards employment

  • Social and recreational needs

  • Rebuilding links with friends, family and the community

Autism

Through listening to service users with Autism and having guidance from their professionals, family and friends, we are able to develop and gain a better understanding of how we can best support them. This enables us to establish an individualised Autism Profile for each service user.


Where possible we encourage clients to gain self-awareness and insight of their Autism. By focusing on reducing triggers, which cause stress, we ensure their comfort and wellbeing. We aim to recognise and understand what is important to each individual, so we can help them to enjoy the things in life that bring them joy and make them feel positive. With our support clients develop confidence so they can take advantage of opportunities to live a happy and fulfilled life.  


Our staff acknowledge the impact and ongoing anxieties caused by sudden change. To reduce the risk of stress and discomfort, we use an appropriate scheduled planning process which provides stability.

Person Centred Planning

Each client is cared for in accordance to their wishes and based on a person-centred

support plan. This makes staff aware on how the service user wants to live their life now and in the future. We ensure clients are always closely involved in the development of their own support plan and  include their own wishes.

Specialist Support

Through ongoing Key Worker engagement, we establish each clients likes and
dislikes, special interests and aspirations.


We support service users with Autism to develop confidence and establish individual goals that we can help them achieve. Examples of activities we encourage them to do include: going to the shops, learning how to cook, learning how to use public transport, gardening and growing vegetables, participating in sports, art or music activities, starting a college course, attending day centres and finding voluntary work. 


The process and pace for each individual service user is different, therefore they may require
additional methods of support. These consist of social stories, teaching them to identify and
understand body language,  understanding facial expressions and educating them on social
norms.

Aquired Brain Injuries

We provide an Acquired Brain Injury (ABI) service providing support for people

with complex needs following a brain injury. We understand that each individual's needs are unique as brain injuries are a diverse diagnosis that may present in many different forms.

 

 Each service user`s recovery pathways and support plans are personalised to suit their specific assessed needs. We also focus on their interests and dreams, which are achievable within their
abilities.
